New Patient Registration
Everyone is welcome in general practice. You do not need proof of ID, address, immigration status or an NHS number in order to register.
However it might help if you have one or more of the following:
- Passport
- Driving Licence
- Birth Certificate
- Rough sleeper's identity badge
If you're homeless, you can give a temporary address such as a friend's address or a day centre.
You have the option to register online or to call into the Practice, to pick up an Application Form, if you have moved into the area.
This process can take a number of weeks until we receive your records, so we ask that you make an appointment for a new patient health check, with one of our practice nurses. This is a 20 minute appointment, that will involve taking your height, weight, blood pressure and any relevant medical history.

Out of Area
Patients who ask to register with us who are outside our catchment area, will be reviewed on an individual basis based on clinical need and if there are children living in the household. Home visits are excluded for patients who live outside our catchment area and shared care and communication between teams can be affected which may result in your application to register being declined.
