Our phone lines are extremely busy at the moment. You can book a GP appointment online or use the eConsult link on our home page.
Our GP appointments are routinely by telephone. If the doctor needs to see you he will arrange an online or face to face appointment. Please ensure we have your up to date mobile and email contacts. A face covering or mask must be worn when coming into the building and during your appointment. Thank you.

If you're a patient at our practice you can now use the new NHS App, a simple and secure way to access a range of NHS services on your smartphone or tablet.
You can use the NHS App to check your symptoms and get instant advice, book appointments, order repeat prescriptions, view your GP medical record and more.
If you already use Mulberry Street Medical Practice online access you can continue to use it. You can use the NHS App as well.

You can choose whether your confidential patient information is used for research and planning. You do not need to do anything if you are happy about how your confidential patient information is used. You can change your choice at any time.
Type 1 opt-out: medical records held at your GP practice - You can tell your GP practice if you do not want your confidential patient information held in your GP medical record to be used for purposes other than your individual care. This is commonly called a type 1 opt-out. This opt-out request can only be recorded by your GP practice. Please fill and return this form to the surgery Type 1 opt-out form
Type 2 opt-out: information held by NHS Digital - Previously you could tell your GP practice if you did not want NHS Digital to share confidential patient information that we collect from across the health and care service for purposes other than your individual care. This was called a type 2 opt-out. The type 2 opt-out was replaced by the National data opt-out. Type 2 opt-outs recorded on or before 11 October 2018 have been automatically converted to national data opt-outs. There is wheelchair access to the building via a ramp, the GP practice is based on the ground floor. A disabled WC is also available on the ground floor. Disabled parking is available within the staff car park. Otherwise free parking is available in the nearby leisure centre.
